(mdebug__output): New variable.
(SET_DEBUG_FLAG): Unset mask if env_value is '0'.
(m17n_init_core): Handle MDEBUG_ALL and MDEBUG_OUTPUT_FILE.
(m17n_fini_core): Close mdebug__output if it's not stderr.
